A cautionary tale that illustrates the importance of accuracy when filling in the entry and of course the DS260. IN this DV2015 case a simple mistake costs Green Card for the selectee in Ghana.
Let’s call this gentleman John Smith. John attended his interview a few days ago and was denied his visa for a simple mistake that I believe to be nothing more sinister than a genuine error. The CO denied him because his original eDV entry (not the DS260, the online entry itself), showed him as having no first name – just a middle and lastname. In reality his firstname was in the middlename field.
The gentleman sent me some screenshots of his eDV confirmation page and his 1NL (the “you have been selected” letter).
The eDV shows his entry as Smith, John – exactly as you would expect.
The 1NL has his name (above the address and after the “Dear”) as “NFN John Smith” and “Dear NFN John Smith”.
Check out this image. There is a checkbox under each name, and the NFN is printed when someone has selected “No First Name”. These fields are process by computers, so I would be certain that the selectee made that mistake, KCC don’t retype anything. The Confirmation on the entry makes sense (i.e. not showing the firstname), and the first clue of the error was the NFN (No First Name) on the 2NL. However, it is OBVIOUS that is you provide a middle name, and say there is no first name, then the name provided as the middle name is actually your firstname.
However, all submitted cases are checked for duplicate entries, so they may believe that John Smith tried to enter with John as the firstname and John as the middlename (in a second entry). The CO could have put him on AP and put the case back for checks on John Smith, but for some reason he decided not to do that. The embassy/CO was particularly harsh, and the embassy is one (Accra, Ghana) that shows up as one with very high denial rates as shown in my league table .
What could he have done. Well if he had understood the critical nature of seeing NFN on his 1NL he could have contacted KCC and he could have possibly have got them to re-run the background checks prior to scheduling his interview. It might have delayed things but it would have diffused the issue somewhat. KCC would not have corrected the error itself, but they would have passed the case to the embassy with a file note explaining that both names had been checked. As it is, the selectee has lost his chance for the Green Card and spent money on the medicals and processing fee – not a small amount of money in Ghana.
The lesson for the rest of us is to take these issues seriously. I read time and time again stories from people such as “my brother knows someone that had that problem and he got away with it, blah blah”. Sure, some cases will be accepted with errors, and some won’t – but why take that risk on what is such a simple process if we just follow the instructions and take advice from genuine experienced people.
simple mistake costs Green Card

Hi Dear Simon
I have a question. My wife is the main applicant and the winner of dv-2015.Her CN is about 2015ASoooo49**and we are from Iran.I have just recently found that While I wanted to write her full name in the first application form I mistakenly wrote one part of her family name(which has 2 parts)in the box of middle name, and unfortunately I unknowingly made the same mistake while I was writing her name in DS-260 on line form. Now I am really concerned about this.I personally believe this not wise if I request for unlock…….Please give me a piece of advice.
March 1, 2015 at 00:29
Well, if you are determined not to unlock there is no advice I can give you. In that case all you can do is hope the CO is understanding.
March 1, 2015 at 05:17
I do thank you anyway………I want to know what the chance of this case number(49**)is in Iran……..Thank you….
March 1, 2015 at 15:07
49xx will be current in May
March 2, 2015 at 18:20
Thanks for the first reply which I’ve understood well. I want to know when our case number will be called… 28*** because it been more than two months we send our Ds260 form… If not is it advisable to call kcc or email them so they can scheduled appointment for us this month???
March 2, 2015 at 21:08
There is no point you calling. KCC are taking around 4 months to process the DS260 forms at the moment – so April interviews have been scheduled for people that are current AND have submitted their forms in September or before. So – if you submitted your forms in December, it will be a while yet before they are processed.
March 11, 2015 at 22:09
Hi. My husband and I have a situation on hand which I want to share with you so we can get advice.. My husband Little niece which is four years came to spend vacation with us and the Ebola crisis stock her with us because no family were visiting each other. The little girl mother was a nurse, unfortunately for her she got in contact with the Ebola virus and die, in one week her father which is my husband biggest brother also die and their two other kids die too left with only the little girl that is with us. Sometimes she sit very sad all by herself when I asked her what’s the matter with you she can say she want to see her parents. She wants them to come and visit her. But I can pet her and make her laugh than she for get. Now she used to me like crazy and I used to her as well. She calls me mommy. But now we will soon be leaving the country and really wish to take her along. We don’t know if we can explain this at our interview? Or adapt her as our child?? This is our worry Every day. We don’t know what to do. We really need an advise.
March 11, 2015 at 22:31
Does your country have a formal adoption process? If so – you should go through that process now. You should also compile all the evidence possible showing the child is an orphan and a blood relative. In reality, there are rules about when an adopted child becomes recognized for immigration purposes – so the sooner you can get adoption papers (and even papers showing you have already been the child’s guardian for some time) the better.
For immigration purposes USCIS have this standard –
Adopted Child: An unmarried child under age 21, who was adopted while under the age of sixteen, and who has been in legal custody and lived with the adopting parent(s) for at least two years.
Legal custody means court agreed.
It seems unlikely that you will have had the child for long enough by the time of the interview – but in the circumstances the CO may accept the child as adopted – if you can provide enough evidence.
However, that is by no means certain. What is more likely is that you will get your visa for you and your husband. You would then have to wait until she is recognized as your child and sponsor her for a Green card – which will take a year or two. If you enter and activate your GC as late as possible – and then apply for a re-entry permit you may be able to stay with the child for most of that waiting period. You will need a lawyer and the whole thing could be costly.
I hope that helps.
March 13, 2015 at 01:18
Thanks. We will start the process.
March 15, 2015 at 10:33
I think I might have made a somewhat similar mistake in my original application.
I have two middle names but I only entered the first one since I never use the second one (forms often don’t have space for two middle names so it is easier just to use the first one). Only problem is my passport lists both names so I put both middle names on the DS 260.
Would you recommend contacting the KCC to explain the error or am I stressing over nothing?
March 15, 2015 at 23:23
KCC cannot “fix” that, so address it in the interview and yes you are stressing for nothing.
May 19, 2015 at 23:33
Hi, I have somehow paranoic question: is unlocking document badly seen? Can unlocking document, and fixing real mistakes, cause any trouble at interview?
May 20, 2015 at 01:40
No. And it is a lot better than going in with mistakes!!
June 8, 2015 at 08:21
Hi Brit, I made a mistake in my d260 as I put my school as senior school instead of secondary school as indicated on my documents, please advise
June 8, 2015 at 15:27
Correct it – email KCC to unlock.
June 8, 2015 at 19:20
Hae i want to remove an applicant i addedas,my spouse can that cause me problems
June 8, 2015 at 19:21
It might do. To remove an applicant you have to contact KCC, but of course, the CO will want to know why you have done that.
July 21, 2015 at 07:41
what if I say I mistakedly added my boyfriend
July 21, 2015 at 10:36
You can say whatever you like – but they are trained to look for people trying to cheat the system….
June 11, 2015 at 08:12

BUT MY CASE NUMBER IS 69XX IT AND MY RESULT WILL COME JUNE OR JULY NEXT YEAR 2016
June 18, 2015 at 09:52
In dv entry i put my First name, last family name and middle name. Now problem is in middle name.In my county (Serbia), middle name, father’s name and not so much in use. The title of my DS260 form stands my surname, first name and middle name. In the form I have written only name as we wrote in the passport with no middle name. Do i contact kcc that we change this or leave but the interview to explain?
June 18, 2015 at 13:47
The DS260 should have the name that matches your birth certificate and passport.
June 25, 2015 at 14:45
i have filled the DV lottery 2015..i made a mistake while filling  …i have written my Last Name in place of first and First Name in place of last… who know I have chance win visa?
June 25, 2015 at 14:49
Did you correct that in the DS260? If you did, you may be OK.
June 25, 2015 at 15:08
Yes, in the DS260 I wrote correct Last name in place of Last name, First name in place of First name ,like my passport
June 25, 2015 at 15:48
OK – well that is the best you could do. Good luck!
June 25, 2015 at 15:22
Simon can I don’t worry?
June 25, 2015 at 15:55
You still have a chance of refusal for the mistake – only the CO can decide.
June 26, 2015 at 03:46
Hi britsimon,
I am one of the selected candidate of edv 2015 and I submitted all my documents to kcc.but today I found my date of mistake. That’s why, I want to request u to provide me some hints to recorrect my form.sir, how can I recorrect it? I hope u reply to my query soon.

July 1, 2015 at 21:46
That is probably too late now – so you will deal with it at interview.
July 1, 2015 at 14:05

You probably have no time left to correct that mistake. You will have to explain at the interview.
July 7, 2015 at 15:27
dear Simon Im sory , Im not selected candidate 2015, Im 2016), that is why I have time correct my ds 260. Can I now e-mail KCC to unlock my application? Or like this mistake not big problem?
Thanks a lot for your answer!
July 7, 2015 at 18:29
In that case YES email KCC to unlock your form and correct it.
July 4, 2015 at 00:00
hi..simon..am part of ds260 winners in 2014 and have gone though all there is from medicals to the inetrview,,,and during the interview i was refused of my visa..the CO said to me that am being disqualified cos my surname dont match the one on my confirmation letter…initially i made a mistake when filling my form with my picture in 2013..my surname is ENNIN but the one with my picture was ENIN…I was not given the chance to explain but was refused the visa..the CO said am disqualified which for me is tooo harsh cos i have the confirmation letter spelt right n all the other document correct expect that of the winner notice that has the typographical error n yet i was treated like a criminal…how can it be possible?is there a way i can get my visa…?any possible way?can i appeal?if i can how can i get an immigration lawyer?or is there any possible way i can get the visa?my interview was on 2nd july 2015..
July 4, 2015 at 00:10
There is no process of appeal, no lawyer can help. I am sorry – there is nothing you can do, except accept the decision. That is very unfortunate.

July 7, 2015 at 21:24
Hi Simon
I’m DV 2016 winner from Nepal. My case number is between 600 to 700.I want to you a question.my question is my one middle name also goes in last name in entering first. After that I call KCC and embassy in my country they said fill what is right in your passport. Then I submit my DS-260 according to them.Is it right?Thank you.
July 7, 2015 at 21:47
It is important that the DS260 is accurate. Normally the passport matches the birth certificate, except if you changed your name through marriage or something. So – yes, matching to the passport is normally safe unless the passport has a mistake.
July 7, 2015 at 22:36
Thank you very much.
July 8, 2015 at 19:43
Hello Simon.
I would like to start by thanking you about the great work you’ve done here to help people for the DV lottery.
I have a few questions about my case.
– When I registered for the DV lottery 2016, I entered 2 of my given names because I thought these ones were only required for US administration. (I am a French citizen and most people here have 3 given names – not just a first and a middle name.) I asked KCC to unlock my DS-260 form and filled out the form correctly with all the names on my passport. Could it be an issue and a reason for a denial of my visa?
– I have an equivalent of a US high school diploma so I meet educational requirements. I went to college but I didn’t get graduated. It was night class in IT but I couldn’t finished it. (I do have certificates for these courses to show though.) Do I have to mention it on my application?
– My case number is EU 25XXX. When will I be interviewed?
Thank you for your help!!!
July 8, 2015 at 22:41
1. It could be an issue – depends on the CO. However, you did the best you could by correcting it in the DS260.
2. No you don’t have to mention it.
3. Around July next year.
July 8, 2015 at 22:59
Thank you so much for your reply!
Will it be bad for my case? I mean is there something else I can do?
I don’t want my application to be denied just for one mistake…
July 8, 2015 at 23:06
You have done all you can do. Some people DO have their case refused for one simple mistake – but I doubt that will happen in your case.
July 8, 2015 at 23:13
Wow. That seems rough. I hope everything will be fine. Thanks for your help!!
August 15, 2015 at 13:26

Make the ds 260 accurate, but the important proof will be the education up to grade 12.
August 15, 2015 at 14:15

Yes make sure your DS260 has all the education. Email KCC to unlock it.
August 17, 2015 at 17:31
Hi Simon! Im also winner 2016 and when I fill up ds 260 i made mistake, my quastion is ,when can email KCC to unlock form, now KCC not answer my quary yet?
Thanks in advance!
August 17, 2015 at 17:39
It takes them a few days, and you must have included your details correctly.
August 17, 2015 at 17:46
I mean now i can e mail to KCC or after when KCC will send me date of interview?
August 30, 2015 at 19:44
Good day Mr. Simon. First I Want say thanks for your great works to help for us. My question is that, when I fill up application to Green card ,I upload my old foto(3 years before), can you please tell me whether consul disqualify me?
August 30, 2015 at 19:57
Some have been disqualified for photos that are obviously too old. But if you appearance has not changed much it might not be obvious that the photo is old. In that case you would be OK.
August 31, 2015 at 02:57
During interview consul will check date of foto?
August 31, 2015 at 04:48
How?
August 31, 2015 at 04:54
Hello Brit
Please I have realised that the DS form does not allow the use of hyphen in the surname column. eDv form had my compound name correctly indicated on 1 NL but I am unable to use my compound name whiles filling DS form.
How best can I get this resolved.
Thanks
August 31, 2015 at 04:56
A space?
August 31, 2015 at 05:00
Please do you think presenting the name as A B instead of A-B(correct name) or AB will pose an issue.
August 31, 2015 at 15:23
A B is best – but it isn’t a big deal.
August 31, 2015 at 04:56
I think jpg file have date of create
September 6, 2015 at 01:01
Hello Brit,
Probably I am overthinking but I have a doubt, The eDv form only have three spaces for the name:
a. Last/Family name
b. Firs Name
c. Middle Name
So, I specified my first name, my middle name and my first last name. For Example: Bach, Mary Jame
In the DS 260, they are more specific and they asked for SURNAMES, so I put down my two surnames, as it appeared in my passaport. For Example: Bach Olsen, Mary Jane
Hadn’t specified my second surname in the edv is a problem? Or I did it right? they just asked for the first last name in the eDV.
Please clarify.
Best Regards,
Lara
September 6, 2015 at 01:20
Well you did the eDV wrong. But it is good you corrected it on the DS260. Most COs would be ok with that if they don’t think it was deliberate.
September 6, 2015 at 02:00
OMG Brit, now I am very worried, I tried to be very accurated when filling the eDV, in Venezuela everybody has two last name (one from the father and one from the mother) so when the form indicate Family/Last Name (no plural and just one space) I assume it was only one last name (the first one), and I also did the same for my husband and two kids (just one last name in the eDV). In the DS 260, where they asked for Sunames (plural) I added for all of us the second last name. Have you heard about a case like mine? i am very nervous. Regards, Lara
September 6, 2015 at 02:14
My wife is from Spain – and has the same situation. So – we entered both her last names in the lastname field.
Like I said earlier – you made a mistake on the eDV. However, I don’t think it will cause disqualification, mostly because you have corrected it on your DS260 – which means they can get an accurate name check.
September 6, 2015 at 02:18
Thank you Brit for your answer !!
September 6, 2015 at 14:38
Hello Brit,
I have another question. In the DS 260, I wrote down my surnames as they appear in my passport, e.g: GONZALEZ DE PRIETO
PRIETO is the last name of my husband and according to Venezuela law, the wife take the last name of the husband but after her first last name and with the preposition “DE”.
In my birth certificate and my high school diploma my last names are, e.g. GONZALEZ FERNANDEZ because I was single at that moment.
In the DS 260 I punched the answer “NO” to the question: Have you ever used other names (i.e., maiden, religious, profesional, alias, etc.)?
Now I wonder if I had to answer YES to that question and wrote down my full name when I was single, e.g. Maria Andreina GONZALEZ FERNANDEZ
Please advice, I don’t want to delay my interview (my CN was current for Oct, but I didn’t receive 2NL) but if it is necessary I will unlock the DS 260.
Thanks in advance for your support.
Regards,
Lara
September 6, 2015 at 15:14
Yes, you should have listed your maiden name. I would unlock it – even if that means a delay.
September 6, 2015 at 16:38
Thank you Brit, based on your experience do you think that this change can be considered an important change? I mean, if they will need start a new background check with my maiden last name that could required 3-4 months of process time? I just want to have in mind the possible length of time that I will need to wait for 2NL. I am sure this process will teach me the art of patience (which is not my best qualities…LOL)
September 7, 2015 at 05:07
Even if there is a delay – it would not be that long – a few weeks maybe.
September 29, 2015 at 16:50
Hi there, My first question is Im eligible for the DV but my wife does not so can she apply.
secondly next year we had a baby in US do we have tell them if yes what could the consequences.
September 29, 2015 at 18:11
Each principal winner must meet the requirements. So – if your wife wins and cannot meet the requirement, she would be refused during interview.
October 22, 2015 at 15:29
hi britSimion, i just read the advise you mentioned about a guy who missed out his opportunity in ghana. i made a mistake on my edv and ommitted an `E`on my name. You advised me to correct it on my ds260, which i did and summuted. reading this post, “KCC would not have corrected the error itself, but they would have passed the case to the embassy with a file note explaining that both names had been checked. “
When i resubmitted my ds260 after the correction, i didn`t recieve any mail which stated that the name will be recheck again. i recieved a mail that stated that my ds260 has been recieved and will be process.
please i dont know if i have to write KCC to do a recheck on my name so that the can sent a file to the embassy to let them kown i didnt do a multiple entery? should i write to them so that i can recieve a email which that mention that?
please i do look forward to your response.
thanks
October 22, 2015 at 16:07
Since you have modified the DS260, the embassy will have those changes available to them.
October 26, 2015 at 11:30
HI Brit, i made a mistake during my edv.. which is i forgot to enter my middle name..
which is in all my other documents with the same date of birth, face and spelling of names..
I have just send my DS260 form with my full name which is the same as in all my other documents.. i seriously need your advice sir
October 26, 2015 at 13:46
Well MOST embassies will forgive that if there is no concern about intent to hide your identity.
November 10, 2015 at 11:52
Hi Simion. Greetings. Please I’m a little bit confuse and worry after reading your statistics of last year results embassies.
Im a Cameroonian studying in Brazil. Based on the readying you posted,
Brazil: issue (4) Refused (1) AP (0) Ready (15) percent refused (5:26) percent AP (00) percent ready (98.95) Percent Issued (15.75)
Cameroon: issue (1187) Refused (194) AP (129) Ready (579) refused percentage (29.9) percentage AP (6:18) percentage ready (27.72) Issued percentage (56.82)
Please Simion based on your expertise in dv analyzes, where can you advise me to apply for my interview? Here in Brazil or back in cameroon my home country.
NB: i did had an omission on my name. Omitted a letter and you did advise me to correct it on the DS260. I did. I do have a high school diploma from my country.
I did the first submission in September and later unlocked and summited in October. Please base on my statue or the information provided, what is your suggestion or advise?
1 )Its ok to do it here or to change it to cameroon or the outcome may not be too different?
2.) With My case number is AF564, if i have chang the location for the interview its too late to do that or risky to do that probably by ending this month, so as to prepare or book for a fligh trick?
Please I don’t want to miss this opportunity. Need your help. If okay base on my situation as mentioned then I will just wait for the 2nl.
. Thanks very. Much.

Hello Brit,
when filling my DS-260 for my wife (principal). we interchanged her given names and surnames on her passport and DS-260 form.
example, on passport :surname is LWANGATOT given names: EVA
on DS-260 Form :surname is EVA given names: LWANGATOT
her CN is 2016AF42***
Do you think this needs to e-mail KCC to unlock?
our interview will be at Kigali US Embassy and it will be the first time (it was done in Nairobi)
Please kindly advise (mine has no issues surnames and given names are ok)

thanks Brit,
but I had already requested KCC to unlock my form before (in July and resubmitted it on 13th July 2015). I hope this will not cause any problem (or delay)
will this not be badly seen?
Kind regards,

Hi Simion, I am sorry for the inconvenience. I did asked a question regarding changing the embassy location I am still waiting for your response.
Please I do have any question closely to it. If it’s possible to change the embassy location do you thinking that can affect or risky my chances for an interview know that my case number is low and current but due to the last submit ion?
Is there any risk for me changing embassy location now like given to my 2nl letter more delay with can affect my chances?
And lastly, should I call kcc and ask them to unlock the embassy location because I’m unable to change anything from it? The form (ds260 ) is open but the section for the change of embassy is close. What can I do? Thank.
Note : I wish to change it from Brazil to Cameroon. Pls I need your direction.
Thanks

You were waiting while I was sleeping – PATIENCE!
Changing the interview location before scheduling is OK. YOU change the address to one in Cameroon, and email KCC to change the location – they will then pick the embassy near the new address.
